Our client is looking to recruit an experienced and compassionate Care Coordinator to join their dedicated team at a residential care home in PE37, Norfolk.

This is an excellent opportunity for an experienced Senior Care Assistant or Care Coordinator who is passionate about delivering high-quality, person-centred care while supporting and leading a team.

Working closely with the Home Manager, you will play a key role in coordinating care, supervising staff, and ensuring residents receive the highest standard of support throughout the night.

Key Responsibilities

  • Coordinate the delivery of high-quality, person-centred care for residents.
  • Develop, implement, and review individual care plans.
  • Monitor residents' health and wellbeing, responding promptly to any changes.
  • Supervise and support the night care team, conducting regular staff supervisions and identifying training needs.
  • Assist with staff rotas to ensure safe staffing levels are maintained.
  • Liaise with residents, relatives, GPs, nurses, and other healthcare professionals.
  • Maintain accurate care records and ensure all documentation meets regulatory requirements.
  • Support quality assurance, audits, and compliance with CQC standards.
  • Respond confidently to emergencies and safeguarding concerns, ensuring appropriate reporting procedures are followed.
  • Promote a positive, respectful, and supportive culture within the home.

About You

The successful candidate will have:

  • NVQ Level 3 in Health & Social Care, or be working towards achieving it.
  • Previous experience in a supervisory or care coordination role within a care home or healthcare setting.
  • Strong leadership and people management skills.
  • Excellent communication and organisational abilities.
  • A good understanding of residential care practices, safeguarding, and CQC requirements.
  • Experience using electronic care planning and documentation systems.
  • The ability to remain calm under pressure and make informed decisions.
  • A genuine commitment to providing exceptional care and improving residents' quality of life.
